寄生蟲病仍是目前嚴重威脅人類健康的疾病之一。近年全國范圍的寄生蟲感染調查發現,我國人體寄生蟲有229種,其中線蟲35種,吸蟲47種,絳蟲16種,原蟲41種,其它寄生動物90種[1]?;谶@種流行現狀,在醫學寄生蟲學教學中引入臨床案例分析可不容緩。然而,當前寧夏醫科大學臨床專業本科人體寄生蟲課程教學時數嚴重壓縮,總學時48學時,其中理論課20 學時,實驗課(講習課)28 學時,可見課程教學任務很重,在這種情況下,傳統的以教師為主體的滿堂灌已遠遠不能滿足時展之需。只有探尋得力的教學輔助工具以及有效教學方法才能解決問題。
博客(BLOG)是一種新興的交流方式。隨著網絡技術的迅猛發展,博客、微博客等相繼成為重要的信息傳播工具[2]。其教育功能逐漸得到教育工作者的重視并被引入到教學中?;谝陨锨闆r,近幾年來我們教研室開展了適合我校實情的人體寄生蟲學教學改革,成功地將傳統授課方式結合博客輔助案例教學引入課堂,介紹如下。
1教學的實施過程
1.1建立病例庫及精心編寫案例 依據教學大綱和教學內容,認真搜集整理寄生蟲的典型病例,盡可能選擇能與教學內容對應的病例(尤其是西北地區特有的病例) 作為重點內容。積極與周圍醫院聯系,及時收集新病例,實時關注文獻報道的國內及區內新發病例,使病例庫得以更新和不斷完善。
1.2建立以學生為中心的教學博客 具體來講,人體寄生蟲學博客至少可以包括以下內容:
1.2.1課程信息 開學前將本課程的教學大綱、教學計劃以及考核方式等信息上傳到博客上,便于學生從整體上把握本課程。
1.2.2課前準備 每次上課前1w,上傳本次課的教學目標、主要內容、重點難點以及與教學內容相對應病例與問題。教師根據本課程的教學大綱和培養目標,選出典型的4~6 種臨床寄生蟲病例,并對每一個病例提出3~5道綜合性問題,對典型病例的一些問題,利用教材、參考書、教學軟件及互聯網等進行課前準備。
1.3傳統教學結合案例分析教學的實施過程
1.3.1改進授課方式 理論課與實驗課不分開,均在實驗室開展。每周1次課,每次5學時。每次課由一名教師主講,一名教師輔導。
1.3.2改良考核方式 我們以往的考核方式為期末卷面成績占70%,實驗報告20%,平時成績10%。引入病例討論后,需改進考核方式,以反映學生主動學習和綜合應用各科知識解決臨床實際問題的能力。 因此,在理論考試卷面中增加了案例分析 診斷等密切聯系臨床的題例,平時成績主要反映學生分析病例、共同討論時的表現。考核方式改為期末卷面成績占60%,實驗報告占20%,平時成績占20%。
1.3.3課堂教學過程 ①主講教師先用1學時(40min)講本次課所有寄生蟲的形態結構,然后學生用1學時觀察標本并繪制圖,完成實驗報告。主講教師再用1學時講解次課所有寄生蟲的生活史及其致病特點,剩余2個學時用來討論案例及問題。②建立教學小組和討論問題:2012級臨床卓越醫師班共36名學生,以學生自愿結合老師協調方式分為4個教學小組,主講老師和輔導教師各負責兩個小組。針對教師提出的典型病例的一些問題,同組同學進行討論。③老師回答問題及效果觀察:教師對學生的解答內容進行答疑,指出學生答題中存在的問題,教師根據每位學生的實際表現,給出本次課的平時成績。
2教學效果分析評價
實驗組與對照組在同一學期嚴格按照教學計劃進行教學。課程結束后,理論課考試采用標準化試卷筆試,以優秀,良好,及格,不及格四個等級記錄;實驗課成績是10次實驗報告分數的總和,同樣以優秀,良好,及格,不及格四個等級記錄。實驗組除理論課成績和實驗課成績外,還有平時成績。在教學效果比較分析時只采用前兩項,見表1。
表1的分析結果表明,實驗組的理論與實驗考核的優秀率與不及格率均優于對照組。說明案例教學不僅能加深學生對寄生蟲病的理解,而且能夠培養學生處理臨床實際問題的能力。博客輔助案例教學可以激發學生的興趣和學習熱情,調動學生學習的主動性和積極性,培養學生自主學習的能力及發現問題和解決問題的能力。有利于培養學生的綜合性思維以及團隊合作能力。
3結論
人體寄生蟲學是一門與臨床緊密聯系的醫學基礎課。通過博客這種重要的交流和溝通工具,可以彌補課堂教學中無法大量進行案例討論的局限,使課堂教學得以擴展,也使學生能夠隨時表達學習心得以及與主講教師探討某些問題,形成一個大家共享的學習平臺。
總之,借助先進的信息傳播工具,在傳統的理論教學的基礎上結合病例討論,是適應現代醫學教育模式的需要,也是提高教學質量及醫學人才實踐能力的迫切需要。
