Until now I’m not sure if I really have interest in teaching, although I, as a free normal student, am supposed to be a teacher in the future. Once I dreamed to be a businesswoman or an official, while it seemed that the choice, which I made out of having no way out one year ago, changed everything. In spite of that, I still believe a promising future is waiting for me. What I need to do is making a new plan for it and keeping the pursuit of it.
It’s clear that I have to teach as a volunteer in the country for two years after my graduation. Maybe I will get on with the children in the JingGangshan, the poorest area in Jiangxi for the years. Being knowledgeable and full of energy, surely I will make a difference there. Knowledge is strength, especially in the competitive society today. Therefore during those two years I decide to proceed my study as a postgraduate. Some years later, I may quit teaching unless I find I’m real interesting in teaching.

在醫院中，消毒供應室是一個重要的輔科室，主要工作內容是為醫院其它各個科室提供無菌物品，并且對器械進行清潔、消毒、滅菌、包裝等，以保證器械的重復使用[1]?？梢哉f，消毒供應室是整個醫院里病原體最多、感染風險最大的科室，因此應該加強感染控制工作。伴隨著醫院規模的擴大，醫務人員數量增加，消毒供應室的工作質量影響到醫院的醫療服務質量。該科室工作具有重復性、工作量大的特點，因此對工作人員提出了較高的要求[2]。我院在消毒供應方面存在工作缺陷，本文對各個工作環節提出改進的措施，取得了良好的效果，結果報告如下：
1 資料與方法
1.1 一般資料 我院消毒供應室內共有12名工作人員，年齡在26―45歲之間，平均年齡34.5±2.1歲;工齡在3―18年之間，平均9.2±1.6年。在本次研究期間，沒有人員流動情況發生。
1.2 改進方法
1.2.1 人員培訓 對消毒供應工作人員進行分層次的培訓學習，主要學習對象是消毒供應的管理規范，同時輔以感染管理規范和消毒技術規范。在這些學習教材的指導下，培訓過程中采用問答、講解、討論、布置任務的形式，來使工作人員對各項技術操作和評價標準進行理解和掌握。最終的學習目標，是使這些工作人員能夠認識到消毒供應科室的重要性，明確工作質量影響到護理情況、感染情況，以及患者的生命健康情況。為了提高培訓學習的效果，可以適當采用競賽、考核的方式，將成績作為評優依據，從而避免人員產生懈怠的心理。
1.2.2 布局改善 所謂布局改善，就是對消毒供應科室內部的職能進行劃分，具體來說，要有清洗消毒區、滅菌區、包裝區、無菌物品存儲區、污物回收區、發放區、生活區等。各個區域之間要進行隔離，通過傳遞窗口進行物品的傳遞，從而盡量減少區域間的空氣流通。另外，不同區域承包到人，開展日常管理工作，每月月底進行大檢查，工作符合要求的進行表揚，不符合要求的進行批評教育，甚至是罰款處罰。這些款項用于科室內部的集體活動，既提升了工作質量，又能夠促進科室團結。
1.2.3 回收、清洗工作改進 各科室使用過的器械，工作人員要對其進行及時的回收與清洗，清洗要包含機洗和手洗兩種方式。其中，機器清洗用于大多數常規器械，手工清洗則是用于比較精密復雜的儀器，以及重度污染儀器的初步清洗。具體的清洗流程是：第一步，預洗;第二步，加酶主洗;第三步，兩次漂洗;第四步，干燥處理。對于管腔類物品和穿刺針，要先使用超聲清洗機，然后手工清洗。對于可拆卸機械而言，要將各部件拆開后清洗，結構比較復雜的地方要多次沖洗。
1.2.4 包裝規范改進 包裝一般使用紙塑或棉布。使用棉布進行包裝時，使用前、后都要進行清洗。如果棉布出現破損、毛邊、補丁、發軟等情況，就要及時扔掉，換用新的棉布。包裝不要過大過緊，保證器械質量在7kg以下，物料質量在5kg以下，幾何尺寸為30cm×30cm×50cm最佳。使用紙塑進行包裝時，密封寬度要大于6mm，機械距離封口之間的距離要大于25mm。同時要確保包裝正確，對滅菌日期和失效日期進行清晰地標注，同時簽署查對人和責任人的姓名。
1.2.5 加強滅菌力度 對于滅菌人員，應該具有較高的專業技能，能夠發現工作中的異常情況，并且采取措施及時處理。在使用脈動滅菌器的時候，要在滅菌前進行B-D試驗。相關物品在裝載的過程中要按照規范進行，保證物品在柜室容積的5%―90%之間。已經滅菌的物品和沒有滅菌的物品要分開放置，并且做好標記。消毒人員要仔細關注滅菌過程，完成后對滅菌效果進行檢查。
1.2.6 質量監測 為了進一步提高消毒滅菌的質量，應該對滅菌爐進行工藝檢驗和化學監測，并且每周實行一次生物監測。對滅菌器材進行大規模修理之后，在使用前要多做幾次B-D試驗。另外，對于物體、空氣、手部每月進行一次細菌培養，若檢測不過關需要查找原因并解決問題。一次性物品要進行抽樣檢測，合格后才能用于臨床。
2 結果
對消毒供應室進行區域化改進方案之后，該科室的工作質量有了明顯的提升，各科室的消毒物品供應更加及時，大大降低了工作差錯的出現。另外，工作人員的積極性得到提高，構建出融洽的工作氛圍，實現了高效率、高質量的工作目標。
3 討論
消毒供應室是醫院中一個重要的部門，負責所有科室器械的清潔消毒、供應無菌用品，同時控制感染。這種工作性質決定了工作人員會接觸到多種病原體，況且消毒過程中環節多、工藝繁雜，容易出現工作失誤，產生缺陷[3]。只有提高消毒供應室的工作質量，才能提升整個醫院的服務水平。就我國目前的發展形勢看，消毒供應室工作人員普遍缺少專業知識，相關工作技能并不過關，對于滅菌、消毒的概念并不清楚。因此，一方面從思想教育入手，讓員工了解操作規范以及該科室工作的重要性，從而提高他們的責任心，在操作過程中融入安全意識[4]。另一方面，加強工作管理和監測，才能夠有效控制感染。根據科室實際情況制定出具有針對性的管理制度，采用獎懲措施來提高工作人員的積極性。只有這樣，才能最終收到滿意的效果。
綜上所述，對消毒供應室的工作進行區域化改進，能夠減少工作缺陷，提高消毒供應室的工作質量，最終促進醫院整體服務水平的提升。
