【Abstract】 Objective To investigate influence of psychological intervention nursing on postpartum hemorrhage-induced psychological states of anxiety and depression. Methods A total of 70 puerpera with postpartum hemorrhage were randomly divided into observation group and control group， with 35 cases in each group. The control group received conventional nursing， and the observation group received additional psychological intervention nursing. Self-rating depression scale （SDS） and self-rating anxiety scale （SAS） were applied to evaluate psychological states in puerpera and effect by psychological intervention nursing. Results There was no statistically significant difference of SAS and SDS scores in the control group before and after intervention （P>0.05）. The differences of SAS and SDS scores in the observation group before and after intervention all had statistical significance （P<0.01）. After intervention， the SAS and SDS scores between the two groups had statistically significant differences （P<0.01）. Conclusion Implement of psychological intervention nursing for puerpera with postpartum hemorrhage can remarkably improve their anxiety and depression. It is worth widely clinical promotion and application.

產后出血（postpartum hemorrhage， PPH）是指胎兒娩出后24 h內失血量≥500 ml， 剖宮產時≥1000 ml， 是分娩期的嚴重并發癥[1]。產后出血其主要原因有子宮收縮乏力、胎盤因素、軟產道裂傷及凝血功能障礙等[2]。出血不僅導致產婦發生生理變化， 還對產婦的心理產生巨大影響， 如易產生煩躁、緊張、焦慮、抑郁、恐懼等負性情緒；不良的心理反應容易加重病情， 尤其是焦慮、抑郁情緒可以使產婦出現產后心理障礙， 嚴重的會出現產后抑郁癥。心理干預護理是以心理支持為基礎， 將應激處理、健康教育與應對技巧有機結合， 鼓勵患者說出所想的問題和心理矛盾， 從更深層次改變其錯誤認知， 改變其對疾病不正確的觀點， 教會患者正確的應對方式和技巧， 幫助患者分析和認識存在的問題， 提出解決問題的途徑， 提高患者的主觀能動性和適應應激能力， 積極應對各種不良刺激， 從而減輕抑郁、焦慮癥狀的發生[3]。對于產后出血產婦， 加強心理干預護理， 可以減輕其焦慮、抑郁等負性情緒， 有利于促進疾病的康復。本文對70例產后出血患者分組后分別實施兩種護理模式， 觀察其在改善產后出血所致產婦焦慮和抑郁方面的影響?，F報告如下。
1 資料與方法
1. 1 一般資料 選擇2013年1月～2014年12月三明市某三甲綜合性醫院婦產科產后出血的產婦70例， 平均年齡25.5歲；初產婦48例， 經產婦22例；順產52例， 剖宮產18例；出血原因：宮縮乏力51例， 產道損傷10例， 胎盤殘留9例；排除有傳染病史及凝血功能障礙、精神障礙產婦；將70例產婦隨機分為觀察組和對照組， 各35例。兩組產婦年齡、文化程度等一般資料比較， 差異無統計學意義（P>0.05）， 具有可比性。同時， 產婦及家屬均同意參與該研究， 該研究得到醫院倫理委員會審批通過， 符合倫理要求。1. 2 治療方法 兩組產婦均按產后出血治療原則進行治療， 即針對出血原因迅速止血、補充血容量、糾正失血性休克、防治感染等。
1. 3 護理方法 對照組給予常規護理；觀察組在常規護理的基礎上增加實施心理干預護理， 具體方法包括：①評估心理狀況及實施心理干預：經積極治療， 病情穩定后應詳細評估產婦的一般情況及觀察情緒狀態的變化；詢問產婦對本次分娩的期盼和實際體驗；回憶分娩情景的頻率；對分娩的感覺；觀察母嬰之間接觸和交流情況；重點了解產后出血產婦有無焦慮、抑郁情緒， 針對發現的問題給予相應的心理干預護理。②建立良好的護患關系：加強與產婦的溝通， 聆聽產婦的傾訴， 理解產婦的感受， 了解其所擔心的問題及其程度， 給予針對性的解釋。用產婦能聽懂的語言進行交流， 語言親切， 態度和藹， 不斷給予精神上的安慰， 鼓勵和表揚產婦。要尊重產婦并給予同情， 不向產婦提要求或強制其做決定， 并接受各種行為表現。應耐心的向產婦講解治療及檢查的目的， 通過交談、撫摸、握手等方式轉移產婦的注意力。盡量滿足其合理要求， 讓產婦積極配合治療護理工作。③實施認知行為干預：因大多數產婦缺乏必要的常識， 受傳統觀念的影響， 重視新生兒性別， 易出現情感脆弱、焦慮等不良情緒[4]， 所以應側重改變產婦的錯誤認知， 指導其進行放松訓練， 并針對其存在的困惑進行疏導， 讓產婦消除顧慮， 放松心情， 精神愉快， 使其樹立樂觀自信的心理， 積極配合治療[5]。④爭取家庭社會支持：家庭支持可以改善患者的負性情緒， 醫護人員應主動給予產婦關愛與關心， 使其增加安全感。同時， 爭取家屬關心、照顧及支持并做好出院指導。兩組產婦分別在分娩后第1天（干預前）及出院前1天（干預后）評估其焦慮、抑郁程度。
1. 4 觀察指標及評價標準 使用Zung開發的SDS和SAS對兩組產婦在心理干預護理前、后焦慮程度以及抑郁程度進行評測。評估兩組產婦干預前、后的焦慮、抑郁程度。
1. 5 統計學方法 采用spss20.0統計學軟件處理數據。計量資料以均數±標準差（ x-±s）表示， 采用t檢驗；計數資料以率（%）表示， 采用χ2檢驗。P<0.05表示差異具有統計學意義。
2 結果
干預前觀察組和對照組SAS、SDS評分比較， 差異無統計學意義（P>0.05）；對照組SAS、SDS評分在干預前、后比較， 差異無統計學意義（P>0.05）。觀察組SAS、SDS評分在干預前、后比較， 差異均有統計學意義（P<0.01）；干預后兩組SAS、SDS評分組間比較， 差異均有統計學意義（P<0.01）。見表1， 表2。
3 小結
對于產后出血產婦除針對出血原因迅速止血、補充血容量、糾正失血性休克、防治感染等綜合治療措施外， 護士應積極實施相應的護理措施。心理護理為其中一項重要的護理措施， 產后出血產婦比較容易出現心情壓抑、沮喪、情緒淡漠， 甚至焦慮、恐懼、易怒心理， 這些心理問題不同于生理疾病， 可以理解為是正常生理活動中的局部狀態異常， 具有明顯的偶發性、暫時性， 常由一定的情境誘發， 當脫離該情景之后， 個體的心理活動則會恢復正常[6]， 只有少數產婦會導致產褥期抑郁， 針對產后出血產婦給予心理支持、咨詢與干預， 解除致病的心理因素， 為產婦提供更多的情感支持及社會支持， 指導產婦對情緒及生活進行自我調節等心理干預護理措施， 可以幫助產婦擺脫出血或分娩產生的焦慮和抑郁等心理狀態。該研究結果顯示， 對照組SAS、SDS評分干預前、后比較， 差異無統計學意義（P>0.05）。 觀察組SAS、SDS評分干預前、后比較， 差異均有統計學意義（P<0.01）；干預后兩組SAS、SDS評分比較差異均有統計學意義（P<0.01）。
綜上所述， 通過主動地與產后出血產婦溝通， 有效地實施心理干預護理， 可以較好地緩解其焦慮、抑郁情緒， 對促進產后出血產婦早期康復具有積極的意義。
